An exciting opportunity within the North East Essex Dementia Service involves working in the Memory Assessment Service alongside the Care home liaison team.
The successful candidate will deliver a high‑level service for patients experiencing dementia and frailty, providing expert clinical assessments, diagnostic support, and post‑diagnostic interventions. The role works within a multi‑disciplinary team of nurses, doctors, OT’s, clinical and assistant psychologists, and will develop dementia care pathways and service improvement initiatives.
- Direct line management of core team professionals.
- Work closely with senior management and senior community team colleagues.
- Support regular administration of community team services to guarantee delivery and development.
- Liaise with local services and other EPUT teams to promote learning and ensure safe and effective service delivery.
- Positive role‑model and deliver leadership underpinned by professionalism, compassion, and empathy.
- Develop and maintain a positive culture that promotes staff welfare and wellbeing.
- Provide effective and regular supervision (1‑to‑1 support), appraisal, and support of mental health practitioners from a mixed professional background. Supervision includes reviewing record‑keeping standards such as assessment updates, care plans, risk‑management plans, and other relevant clinical documents.
- Promote a safe and effective learning environment that facilitates continued professional development of staff and students.
- Demonstrate skills in negotiation, persuasion, reassurance, and motivation.
- Report to the relevant Line Manager to ensure clear communication.
- Have excellent communication skills, including verbal, written, and presentation skills.

- Lead the Memory Assessment Service, a age‑inclusive service providing assessment and diagnosis for people with memory problems and other cognitive difficulties.
- Support the care home liaison team in providing care to residents with memory difficulties or dementia.
- Collaborate with relevant parties to prevent unnecessary admissions to general hospitals and older adult mental health inpatient services.
- Provide effective clinical leadership for a multidisciplinary team within available resources.
- Coordinate general functioning of the team, manage performance, and monitor quality of service.
- Line‑manage and supervise staff, ensuring proactive assessment, monitoring, and evidence‑based care.
- Evaluate strengths and areas for improvement in collaboration with the Clinical Manager and senior team, and develop improvement strategies.
- Drive transformation of community mental health services in line with the North East Essex strategy.
- Professional qualification in nursing, social work, or occupational therapy.
- Ability to deliver key therapeutic interventions: CBT, psychosocial interventions, family interventions, DBT approaches.
- Master’s degree or willingness to undertake one.
- Knowledge of working with external agencies to support the service user group.
- Ability to supervise and conduct complex assessments and reviews of care in line with mental health care cluster requirements.
- Provide clinical leadership to help the team meet performance and contractual targets.
