Colchester Medical Practice has an opportunity for a care co-ordinator to join our team on a full-time basis working 37 hours per week over 5 days. This role is based at our Wimpole Road Site.
The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients.
Main duties of the job- Working within our proactive care team, following up with patients who have recently been discharged from hospital.
- Working with GPs and other professionals within the practice to identify and manage a caseload of patients. Particularly working with vulnerable patient groups with long-term conditions that may require this extra support. This may include working with individuals, their carers, families and healthcare teams.
- Using in-depth knowledge of locally available support and services todevelop care plans with patient, focusing on prevention.
- Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
- Helping patients prepare for, or follow-up after, clinical conversations.
- Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date.
- Dealing with day to day admin re safeguarding.
Colchester Medical Practice is a well established PCN, delivering excellent care to over 43,000 patients across the city of Colchester. We have four sites (Shrub End Surgery, Castle Gardens Medical Practice, Wimpole Road Surgery and Parsons Heath Medical Practice) and a new health and wellbeing hub, based at our Parsons Heath site. The practice is well established with a rating of GOOD by the Care Quality Commission.

Person Specification Qualifications- Diploma Level 2 in Health and Social Care, or an equivalent qualification
- Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
- Familiar with local resources and services, including how to access them.
- Motivated by helping people, with care and empathy.
- Able to deliver person-centred support in a non-judgmental way.
- Experience of completing holistic person-centred care planning assessments.
- Strong listening and communication skills.
