Job summary

Rowhedge & University of Essex Medical Practiceis currently looking for a Social Prescriber to join our team.

The Social Prescriber will play a vital role in enhancing patient care and well-being by connecting individuals with non-medical support services within the community.

Main duties of the job Collaborate with healthcare professionals to identify patients who would benefit from social prescribing.

Conduct holistic assessments to understand patients' social needs and preferences.

Develop personalized care plans and signpost patients to relevant community resources and services.

Educate patients on available social support options and empower them to take control of their health.

Organize and facilitate group activities that promote mental well-being, physical health, and social inclusion.

Maintain accurate records of patient interactions and outcomes in accordance with data protection regulations.

Essential Experience in promoting wellbeing services to facilitate independence personal resilience.

Experience in creating wellbeing assessments that take into consideration the role and possible impact of the wider determinants of health on wellbeing.

Experience in building relationships with key staff in GP practices , attending relevant meetings, becoming part of the wider network team, giving information and feedback on the role of social prescribing.

Have experience as a Health Coach
