An exciting opportunity has arisen for a Developmental Senior Registered Community Nurse to join our North East Essex Community Nursing Service.
You will be appointed at Band 5 and complete a structured Senior Registered Nurse (SRN) Skill Passport, progressing to Band 6 Community Senior Nurse on successful completion.
This role is suited to a motivated Registered Nurse ready to develop advanced clinical, leadership and case management skills within an integrated neighbourhood team.
We provide nursing care 08:00-18:00, 7 days a week, supporting patients in their own homes. Our focus is on:
- Promoting independence and self-management
- Preventing avoidable admissions
- Delivering high-quality, evidence-based community care
You will work within a multidisciplinary team managing complex and long‑term conditions.
Through the SRN Skill Passport, you will develop:
Advanced Clinical Practice- Complex assessment and case management
- Wound, leg ulcer and pressure ulcer care
- Catheterisation and subcutaneous medication
- Palliative and end‑of‑life care
- Holistic diabetic care and referral triage
- Daily workload allocation
- Supporting junior staff
- Audit, governance and service improvement
- MDT collaboration with GPs, social care and hospice teams
- Risk assessment and autonomous decision‑making
Full training, supervision and competency assessment will be provided.

Main duties of the jobBe responsible for the expert assessment, planning, implementation and evaluation of programmes of evidence based nursing care to meet complex health care needs to individuals and their carers within the defined caseload across the community setting.
To provide ongoing nursing care and support to patients and carers, promoting the role of the nurse and clinical excellence.
To prescribe medicines within a defined clinical area according to the scope of practise and professional competence of the individual.
To support any adult who requires care at home through a diverse range of pathways which support wellness and promote independence where the patients care can only or is best delivered within the home environment.
Assess, review and minimise clinical risk and avoid crisis where possible or take appropriate measures or escalation to ensure risk is both known, explored and understood by the relevant staff.
Embrace an open approach to clinical care which upholds evidence based care and underpins the quality and governance expectations of the organisation.

Experience- Experience of working with professionals from other disciplines
- Experience of working as a member of a team and also able to work autonomously
- Ability to demonstrate good leadership qualities
- Advanced clinical skills with experience in the management of complex/long‑term patient care
- Community experience
- Experience of managing/leading a team
- IT literate e.g. Word processing, internet and e‑mail use
- Able to demonstrate excellent communication skills, both verbal and written
- Professional attitude
- Ability to work flexible hours to meet the needs of the service
- Car driver with valid UK and have access to a car for work purposes
- Ability to work under pressure
- Be able to prioritise tasks and delegate appropriately whilst continuing to manage time effectively
- RGN/RN ADULT, with current NMC Registration
- Evidence of recent CPD
