Job Overview
You will work as a Band 6 Senior Community Nurse with the Crisis Response Team. Your role is to provide high‑quality, evidence‑based, holistic assessments to patients in line with NMC professional code and Trust policies. Assessments are carried out in the community and you must be able to drive; the team uses a lease car.
Key duties include working with the two other teams that form the Urgent Care Pathway – Hospital Liaison and Home First – to respond to urgent and emergency mental health crisis. The team also covers the 111 Option 2 telephone contact service, so part of your role will involve taking calls from service users.
You will be responsible for the day‑to‑day co‑ordination of shifts within the Crisis Response Team and work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi‑disciplinary team to provide a 7‑day, 24‑hour, 365‑day service.
Shifts are 11.5 hours long with a one‑hour break. Ability to work some night shifts is essential.
Main Duties- Provide comprehensive assessment and care planning in line with the Mental Health Care Clusters.
- Produce accurate and timely documentation on all service users in line with professional code and Trust policy.
- Triage referrals to ensure a timely response and accurate allocation according to risk.
- Participate in the planning and ongoing review of service users awaiting assessment or continuing care to ensure effective and safe care.
- Provide practical support, advice, signposting and education to service users, carers and other disciplines.
- Be aware of other statutory and voluntary services to signpost service users for appropriate care.
- Record all patient‑related activity information onto the Paris system.
- Conduct risk assessment of the individual and potential hazards in the service user’s environment.
- Co‑ordinate shifts within the team, ensuring coverage across the 24‑hour service.
- Band 6 nursing registration and experience in community mental health nursing.
- Ability to drive and access to a lease car.
- Availability to work night shifts and a 7‑day service.
- Strong communication, triage and documentation skills.
- Knowledge of NICE guidance, NMC code and Trust policies.
- Current DBS check – the cost is the responsibility of the new employee.
