Specialty Pharmacy Patient Care Coordinator

Overview

The Specialty Pharmacy Patient Care Coordinator (PCC) is responsible to provide full integrated support to patients, pharmacists and clinic staff by providing full continuum of specialty medication services. PCCs are result-oriented with a positive outlook and clear focus on high quality patient care and service. This position requires a high level of detail orientation and strong administrative/organizational skills. This is an on-site position.

• Provide full integrated support to patients, pharmacists and clinic staff by providing full continuum of specialty medication services.

• Education:
  
  High School / GED plus one year of post high school education or related work experience.
• Certifications:
  
  Pharmacy Technician National Certification through PTCB or NHA required.
• Licenses:
  Vermont Board of Pharmacy Technician License required; maintain Pharmacy Technician Registration/License with the State of Vermont Board of Pharmacy in good standing;
  National Certification in good standing.
• Experience:
  
  One year technician experience as a Pharmacy Technician or equivalent required; three to five years preferred; specialty pharmacy or pre-certification experience strongly encouraged.

• Conditional offers of employment are contingent upon favorable completion of a thorough background check and post-offer, pre-employment drug screening. Candidates must submit to a drug screen from a vendor approved by the UVM Medical Center. Refusal to obtain a drug screen within the requested time frame may result in rescission of the offer. If a drug screen is positive, the offer may be rescinded depending on the circumstances.
