We are looking for therapists that are available weekdays.

As a home health physical therapist
, you'll be part of a collaborative interdisciplinary team including RNs, LPNs, LNAs, Occupational Therapists, Speech‑Language Pathologists, Social Workers, and administrative staff, providing comprehensive, patient‑centered care.

As an Adult Home Care Physical Therapist
, you will:

• Perform comprehensive evaluations and assessments
• Develop individualized treatment plans tailored to each patient’s needs
• Deliver skilled therapy interventions to improve mobility, strength, and function
• Support patients in remaining in their homes safely and independently

Our coverage area is segmented into four territories within Chittenden and Grand Isle Counties
. Case load territory is assigned after orientation.

• Bachelor’s degree or higher in Physical Therapy
• 2+ years of general PT experience preferred, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
• Home health experience preferred, but not required; we can train the right person!
• Current Vermont Physical Therapist license, or willingness to obtain one
