Overview

Join our Community Hospice team as a Case Manager Registered Nurse to build lasting connections with patients and their families. We provide compassionate, expert care that allows individuals to remain comfortably  a key member of our interdisciplinary team, you will deliver skilled nursing care, educate patients and families, and support decisions that honor their wishes.

Responsibilities
  • Deliver skilled nursing care in the home setting as part of an interdisciplinary team.
  • Educate patients and families about care plans, treatments, and end-of-life preferences.
  • Create a supportive environment that honors patient wishes and promotes quality of life at home.
Requirements
  • Current RN licensure recognized by the State of Vermont.
  • 2 years' experience in home health, home hospice, or med surg, or a combination of these settings.
On-call

On-call is not typically required; it is variable depending on program needs.
