RN - Home Health

Chittenden County and Grand Isle County, VT

This is a 13-week contract position.

Shift: 8am - 5pm, Monday to Sunday with a requirement of one weekend per month.

The Registered Nurse will provide skilled nursing care to clients in their homes. This role requires strong clinical assessment skills and the ability to navigate various Electronic Medical Records (EMRs). The nurse will manage a caseload of 22-28 clients, performing routine visits, admissions, recertifications, and discharges.

• Conduct comprehensive patient assessments and create care plans.
• Provide skillful nursing interventions including wound care, medication administration, and patient education.
• Document patient care in the EMR system (Netsmart) accurately and timely.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Adhere to safety and regulatory standards.

• Minimum of 1 year of clinical experience in home health care.
• 3-5 years of home health experience preferred.
• Fluency in OASIS and familiarity with Netsmart EMR is preferred.
• AHA BLS certification is required.
