Nurse Practitioner PCN

Cold Lake PCN is seeking a skilled Nurse Practitioner to work within a collaborative care team focused on holistic patient wellness. Key Responsibilities:

• Manage patient care with chronic conditions effectively

• Develop individualized health care plans with physicians

• Lead and facilitate patient navigation for optimal outcomes

• Engage in facilitating mental health group programs

• Collaborate on quality improvement initiatives within the team

Requirements:

• Master’s degree in nursing with NP certification

• At least 3 years of nursing experience in primary care

• Valid BCLS certification for health care providers

• Experience with Indigenous populations is preferred

• Strong decision-making and critical thinking skills
