Mid-Level Social Worker

Join AHS as a mid-level Social Worker II in Cold Lake, Alberta. Contribute to patient care coordination, resource navigation, and psychosocial assessments in a collaborative healthcare setting.

This full-time position supports patient transitions across Acute Care, Home and Community Care, and Continuing Care Homes in Cold Lake and Bonnyville. You will provide essential clinical social work services, including financial assessment, counseling, and discharge planning. Ideal candidates should possess strong clinical judgment and communication skills while working independently or in a team environment.

Key Responsibilities:

• Provide psychosocial assessments and resource-based interventions

• Conduct individual, group, and family treatment sessions

• Assist with discharge planning and case management

• Support patient transitions between care facilities

• Engage in public awareness and preventive activities

Requirements:

• Bachelor’s degree in Social Work

• Active registration with Alberta College of Social Workers

• Driver's license and reliable vehicle

• At least one year of experience in relevant care settings

• Knowledge of community supports and AHS navigation
