Job Description** Must be a valid driver**
This position provides a personalized blend of in‑home and in‑community services that focus on skill development and independence. You will work with multiple people served, use your own vehicle for transportation, and receive mileage reimbursement.
** Not a residential group‑home setting**
Wage & ScheduleWage: $18.50
- Wed: 11 am‑4 pm
- Fri: 9 am‑3 pm (11 hours/week)
- Schedules are subject to change based on individual needs
- Assist individuals in 1:1 in‑home and in‑community settings to pursue a happy, healthy, and independent life.
- Support daily activities such as personal care, meal prep, transportation, housekeeping, and laundry while fostering independence.
- Maintain a clean, safe space that encourages personal goal achievement.
- Provide skill development in community participation, health, safety, wellness, household management, and adaptive skills.
- Examples: escort person served to errands and appointments, teach budgeting skills, and integrate into social events.
- Attend team meetings, write progress reports, and assist supervisors in maintaining consistency and communication.
- Create lasting, positive impacts through coaching, mentoring, and encouraging goal attainment.
- Minimum age 18 (or 21 depending on the person served)
- Some post‑secondary education, training, or experience in a related area.
- Basic computer skills.
- Responsible, reliable, and able to work independently.
- Valid driver’s license, acceptable driving record, auto liability insurance, and a reliable vehicle (required)
- Willing to drive within a 30‑mile radius.
- Adept with technology and possess a smart device with adequate data.
- Flexible team player who provides resources and positive support.
- Strong communication skills in reading, writing, and documenting care.
