Overview

We provide in-home and in-community supports to enhance independence and life skills for individuals with intellectual and developmental disabilities, including those with mental health, behavioral, and complex medical needs. We operate across fifteen states, delivering flexible, high-quality services through Medicaid and Community Support Waivers.

Responsibilities- Assist individuals in a 1:1 in-home and in-community setting, supporting them in pursuing a happy, healthy, and more independent life.
- Provide skill development in areas including, but not limited to, community participation, health, safety and wellness, household management, and adaptive skills. Examples include accompanying the person-served on errands and appointments, budgeting skills, and integrating into social events.
- Attend team meetings, write progress reports, and assist supervisors in maintaining consistency in service provision and communication with the support team.
- Coach, mentor, and encourage others to reach their goals to create lasting, positive impacts on lives.
- Varying schedules available
- Hours vary by location and the needs of the person-served
- Schedules may change based on individual needs
- Must be at least 18 years old (or 21 depending on the person-served)
- Education, training, or two years of equivalent experience in working with developmentally disabled and/or mental illness, as determined by the supervisor. Must be responsible, reliable, and able to work without direct supervision.
- Valid driver s license with acceptable driving record, auto liability insurance, and a reliable vehicle to transport the person-served
- High school diploma or GED
- Associate degree in a relevant field OR two years equivalent experience in working with developmentally disabled and/or mental illness
- Willing to drive within a 30-mile radius
- Adept in the use of technology and possess a smart device with adequate data
- Flexible and a team player, able to provide resources and positive support for the person-served
- Good communication; ability to document care, follow support plans, and collaborate with internal and external team members
