Job Summary
The Stroke Program Coordinator/Patient Care Supervisor has primary responsibility is to implement and maintain a system of care for stroke patients and their families throughout the continuum of care. The coordinator/supervisor works in collaboration with the ED nursing leader, ED medical director, stroke medical director, and other members of the health care team and management staff. The coordinator/supervisor has a secondary role to assist the ED leader in the daily operation of the department and to assist in patient care duties as needed.

- Support and adhere to hospital policies, procedures, philosophy and mission.
- Interpret and implement policies and procedures; make recommendations for revisions; assist with updating policies and procedures as directed by the ED manager.
- Participate in the development and planning of goals and objectives related to stroke care.
- Abstract stroke charts and submit data to the state stroke registry.
- Represent the stroke programs on various hospital and community committees to enhance and foster optimal stroke care management.
- Supervise adherence to hospital policies and procedures and standards through observation, medical record review, staff feedback and other appropriate sources.
- Assist with implementation of program initiatives.
- Monitor compliance with statewide stroke and stroke system regulatory requirements.
- Create, monitor, and maintain quality metrics applicable to each program and the emergency department.
- Other duties as assigned.
- Prepare for statewide stroke system designation site visits.
- Participate in state and regional stroke care activities as directed.
- Respond to stroke team activations that occur during work hours; function in whatever role necessary to assist the team in the care of the patient.
- Serve as a resource for the hospital staff regarding stroke care issues.
- Implement strategies for ongoing stroke program development and improvement.
- Collaborate with the ED manager and program medical directors, physicians and other health care professionals to provide clinical and system oversight for the care of patients, ensuring the provision of efficient, quality and cost‑effective care.
- Improve departmental performance.
- Maintains quality control programs and participates in the organization’s overall performance improvement control program.
- Monitor performance of hospital staff involved with the care of stroke patients.
- Monitor stroke patient outcomes; evaluate for trends.
- Coordinate with physicians, nurses, and other in‑hospital staff and outside providers to evaluate and address specific patient care issues.
- Participate in case review.
- Assist the ED manager and program medical directors in the development, implementation, and evaluation of a quality plan that is multidisciplinary and patient‑outcomes focused.
- Assist with identification, investigation, reporting and follow‑up of incidents and quality issues throughout the program while maintaining confidentiality.
- With the ED manager, monitor the stroke team’s availability and compliance with policies and standards. Also, monitor the stroke PI program in collaboration with the ED manager and program medical directors.
- Assist in data collection related to the applicable program’s patients.
- Assist with and analyze registry data on stroke patient populations; identify trends for strategic planning and performance improvement.
- Complete the measurement of selected outcomes for the stroke patient populations.
- Assist with planning, coordinating, and evaluating stroke-related educational programs for nursing staff.
- Monitor physician and nurse compliance with the educational requirements of the stroke programs.
- Ensure that the staff involved in the care of stroke patients meets educational requirements of each applicable program.
- Assist with developing, coordinating and implementing orientation, nursing education, and in‑service programs related to the care and management of stroke patients.
