Position Summary
North Country Home Health & Hospice Agency Physical Therapist Assistant provides physical therapy treatments under supervision of a qualified Physical Therapist. The role utilizes therapeutic exercise, modalities, and activities to achieve the highest level of function possible within the home environment. This position works within a leadership team that maintains patient quality outcomes scoring in the top 20% of Home Health Agencies in the country.
AccountabilityAccountability first to the supervising Physical Therapist and second to the North Country Clinical Nurse Manager.
InterrelationshipsWorks closely with medical staff, clinical staff, and support staff of North Country Home Health & Hospice.
Required Qualifications- Associate degree from an accredited physical therapist assistant program
- Current and valid Physical Therapist Assistant license granted from the New Hampshire Office of Allied Health Professions
- Good organizational skills with the ability to multitask
- Excellence in customer service and communication skills
- Competent in Computer use:
Windows - BLS certification required
- Performs physical therapy procedures in the client’s home, as prescribed by the physician and supervised by a qualified Physical Therapist.
- Administers non‑complex therapeutic exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities to clients with relatively stable conditions.
- Instructs, motivates, and assists clients in learning and improving functional activities.
- Instructs patients and family members on the proper use of prosthetic devices.
- Observes client’s progress and response to treatment and reports findings to Physical Therapist.
- Modifies plan of treatment, as authorized by the physician, after subsequent authorization, instruction, and supervision by the Physical Therapist.
- Responsible for maintaining current progress notes in the patient’s record. Patient progress in relation to goals is noted, type of treatment rendered and evidence periodic reviews of the referring provider also noted in record.
- Ensures that all documentation is legible and in the format approved by the department: SOAP notes for daily visits and a narrative format for an initial evaluation is preferred.
- Ensures that equipment used by the patient is in safe working condition prior to use by the patient and that any unsafe equipment will be reported to the Clinical Nurse Manager.
- Attends meetings as designated by the Clinical Nurse Manager.
- Works with the Clinical Nurse Manager and supervising Physical Therapist to develop new programs and collaborate on specific departmental projects based on patient needs.
- Demonstrates knowledge and skills necessary to provide appropriate care within the patient’s home.
- Demonstrates knowledge of appropriate modification of treatment programs and safety principles for providing care within the home.
- Manages time effectively to ensure that deadlines are met and that personal priorities support organizational objectives.
- Performs all other duties as assigned.
