What You’ll Do:

• Conduct thorough physical assessments and analyze medical histories to assess patient conditions.
• Develop and implement individualized treatment plans tailored to each patient's specific needs and rehabilitation goals.
• Perform therapeutic exercises and training programs to enhance patients' functional abilities and independence.
• Educate patients and their families on injury prevention, wellness, and steps towards recovery.
• Accurately document and monitor patient progress towards achieving rehabilitation objectives.
• Collaborate effectively with other healthcare professionals to ensure comprehensive patient care.

Qualifications:

• Successful completion of an accredited Physical Therapy Program.
• Home Health experience required.
• At least nine months of experience working as a Physical Therapist.
• Excellent interpersonal skills with a compassionate approach to patient care.
