Location:

College Station, TX (Office Location)

Salary: $40 - $42 per hour (offer based on years of experience).

Schedule:

PRN as needed + on‑call support.

Coverage Area:

Qualifications
  • Conduct comprehensive nursing assessments and develop individualized care plans in coordination with the interdisciplinary team (IDG), addressing physical, emotional, social, and spiritual needs.
  • Maintain timely and accurate documentation of care, interventions, and patient status updates, ensuring compliance with internal protocols and regulatory requirements.
  • Coordinate care activities including visit scheduling, medication management, physician collaboration, and interdisciplinary communication to ensure continuity and quality of care.
  • Provide education and support to patients and caregivers, and respond to urgent needs during on‑call or admissions assignments.
  • Supervise LPNs, LVNs, and hospice aides as needed, and participate in IDG meetings, patient conferences, and ongoing clinical education.
  • Support pre‑hospice services by evaluating patient status via phone, telehealth, or in‑person visits and identifying needs for additional care or services.
