Job Description & How to Apply Below

You've spent years caring for clients. Now help lead their care.

Client Care Coordinator | Care Crew Home Care | DFW Metroplex | Full-Time

You've done the hands-on work. You've cared for clients through dementia, decline, and end of life. You've learned to walk into a home and know right away when something is off. Families trust you, and so do other caregivers.

Now you're ready for more.

As our Client Care Coordinator, you'll use that experience in a bigger way. You'll assess new clients in their homes, help build their care plans with our Nurse Practitioner, and be the person families and the care team turn to when something needs attention. You'll still be in clients' homes every day, doing what you do best, but now with more responsibility, more independence, and an NP working beside you as you grow.

Care Crew serves aging adults across Tarrant County and the wider DFW area, with a focus on dementia care, veteran care, and hospice support. If you're an experienced caregiver ready to take the next step, this role was made for you.

What you'll do
  • Assess clients in their homes. You'll do in-home functional evaluations for new clients and build individualized service plans with our Nurse Practitioner.
  • Watch for changes. You'll make regular in-home visits, notice changes in condition, safety, or function, and raise medical concerns with the NP right away.
  • Keep care plans current. You'll update service plans as needs change and make sure what happens in the home matches the plan.
  • Support families. You'll explain our services and what to expect, follow up regularly, and look into any concerns.
  • Back up the care team. You'll check in on caregivers in the field and reinforce Care Crew standards. If a call-off can't be covered, you'll step in and provide care yourself.
  • Take your turn on call. You'll handle urgent concerns after hours and go on site when it's needed.
  • Document accurately. You'll keep records in our EVV, EMR, and CRM systems and report at weekly leadership huddles.
What we're looking for
  • CNA, MA, LVN, or RN,
or an experienced caregiver with supervisory experience
  • A lot of hands-on care experience in home care, hospice, or senior care
  • Confidence and comfort working in clients' homes with all kinds of people, conditions, and settings
  • Strong clinical awareness, including recognizing decline, safety risks, and when to elevate
  • Clear, caring communication with clients and families
  • Valid driver's license and reliable transportation
What we offer
  • Full benefits: medical, dental, vision, disability, and life insurance
  • 40 hours of PTO per year
  • Company-issued phone and iPad
  • Mileage reimbursement, with possible use of a company vehicle
  • A role with real independence and variety, out in the community
  • Clinical backup from a Nurse Practitioner as you grow
