Our organization has over 800 employees and serves more than 74,300 patients annually in the Pikes Peak and East Central regions of Colorado.

Peak Vista serves 14 counties, from the Front Range to the Kansas border, with locations throughout Colorado Springs, Fountain, Divide, Limon, and Strasburg. Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC). This RN supports three clinic locations.

Compensation (Pay)

$32.39 to $46.96 hourly, based on experience. An additional $2.00 per hour if you hold a BSN.

A $5,000 sign‑on bonus: $1,250 after 30 days, $1,250 after 90 days, $1,250 after 6 months, and $1,250 after 9 months of employment. Current employees are not eligible. Loyalty bonus – $5,000 upon 1 year of service and $1,250 for every additional 3 months, prorated for part‑time employees.

Job Summary

The Care Team Nurse functions as a liaison between the patient and members of the Care Team in a joint effort to optimize the patients’ health status and quality of life. The Care Team Nurse will perform routine and emergent nursing functions pertinent to efficient, ambulatory patient care, while following the guidelines set forth by the Colorado Nurse Practice Act.

Essential Duties and Responsibilities
  • Assesses and triages for the appropriate level of care based on comprehensive risk assessment.
  • Assesses patient’s readiness for change, physical and psychosocial needs.
  • Identifies and links patients to available resources, social support systems, family support.
  • Assesses high risk and rising risk patients consistent with current Policies & Procedures for Care Management.
  • Facilitates access to quality and cost‑effective care.
  • Collaborates with and maintains healthy working relationships with community partners.
  • Provides education to patients, families, and caregivers.
  • Encourages patient engagement and self‑management of health.
  • Provides outreach to individuals and the community.
  • Provides in‑reach to patients who are presently inactive in healthcare.
  • Utilizes technology such as Electronic Health Records, state registries, Health Information Exchange.
  • Maintains accurate and complete documentation of all interactions and billing practices.
  • Works with team members across all levels of the organization.
  • Supports the clinical training department and ensures Medical Assistants are competent.
  • Assists providers with inbox tasks, medication refills, and paperwork.
  • Triages and delegates follow‑up for labs and outside testing.
  • Advocates for patients.
  • Provides education on disease processes and medication regimens.
  • Works with care teams to prepare patient charts and review schedules.
  • Understands immunization schedule to support clinic staff and patients.
  • Operates vehicles safely and professionally and travels to Peak Vista facilities as needed.
  • Promotes and demonstrates Peak Vista’s mission, vision, and core values.
  • Develops and maintains relationships with all departments, management, medical staff, and leadership.
  • Embraces cultural diversity in self and community.
  • Responsible for human, financial, and material resources and entrusted data.
  • Strives to deliver the best outcomes and highest quality service.
