Compensation: $85,000 – $95,000.

Paid Days Off: 30 days annually (up to 6 weeks, including company holidays).

• Perform skilled nursing visits and complete coordination of client care.
• Serve as point of contact for all disciplines involved in patient care.
• Oversee the frequency of visits for the episode.
• Consult with the physician and the office as needed, providing details about patient care.
• Perform accurate OASIS collection, ensuring the medication profile remains current.
• Ensure lab values are reported to the physician in a timely fashion.
• Attend weekly case conferences and monthly case manager meetings.

• Licensed as a Registered Nurse (RN) in the state of employment.
• Preferred:
  Minimum of one year of clinical experience.
• Demonstrated knowledge and skill in current nursing practice.
• Valid state driver’s license and automobile liability insurance.
