As a Registered Nurse with our team, you'll provide skilled nursing care to participants in their homes, working alongside an Interdisciplinary Team to achieve meaningful care plan goals. You'll conduct assessments, perform skilled procedures (wound care, medication administration, immunizations), provide medication reconciliation, and deliver patient education—all while building real relationships with our frail and elderly participants and their families.

• Comprehensive onboarding with a dedicated preceptor

• Graduate of an accredited RN program, licensed in Colorado
• 1 year experience with frail/elderly populations preferred (hospital, long‑term care, or home health)
• Strong communication skills and ability to work collaboratively with IDT members
• Comfortable with EHR, email, and electronic timekeeping
• Reliable transportation with current auto insurance

• Starting pay: $38.38/hour (increases with experience)
• 403(b) with up to 5% company match
• PTO from day 1 (with cash‑out options twice annually)
• Annual performance pay reviews
