Catholic Charities is seeking a Family Life Coach to walk alongside families as they navigate challenges and work toward stability, growth, and connection. This highly relational role centers on building trust with families through regular, personalized outreach — including home visits — and helping to identify and achieve goals rooted in strengths.

Position Summary
The Family Life Coach serves as an advocate and connector, helping families access resources, build protective factors, and strengthen their support systems. This role provides strength-based case management and support services to families experiencing instability, with a focus on promoting child safety, family well-being, and long-term stability. Managing a caseload of 15-18 families, the Life Coach engages families through home visits and community-based meetings, developing individualized service plans guided by family voice and a 2

Generation approach. This role requires strong interpersonal skills, a commitment to trauma-informed and culturally responsive practices, and a willingness to grow through ongoing training and professional development. All training is provided during work hours, with opportunities for self-paced learning, continuous feedback, and team collaboration.

Essential Duties
Key tasks and responsibilities to be performed in the role.

  • Reach out to referred families to engage them in program enrollment, offering case management and program services through a variety of outreach strategies (e.g., phone calls, texts, in-person visits) to engage families.
  • Engage and interview families for program enrollment and complete needs assessments using grant-designated tools. Provide strength-based case management services and collaborate with families to set goals using a family-led approach. Conduct regular follow-up to assess goal attainment and referrals needs. Manage a caseload of 15-18 families.
  • Schedule appointments at times and locations convenient for families, creating a warm and welcoming environment. Conduct visits weekly, semi-monthly, or monthly, based on family needs. Build rapport, assess needs, set family-centered and strengths-based service plans, provide direct services, and connect families with resources to address their concerns and priorities.
  • Develop effective case plans with families regarding parenting skills, family issues, economic stressors, parent/child relationships, and community connections. Track measurable progress toward goals, acknowledge family strengths, resilience, and resources. Work toward keeping children safe in their homes and stable in school settings. Promote protective factors with families to ensure child safety.
  • Actively communicate with and seek support from supervisor, including information on referrals received, home visits completed, and allocation of funds.
  • Document agreed-upon next steps and assign responsibility for each task. Follow up with families to assess progress and success with referrals.
  • Advocate on behalf of families with external agencies, such as welfare offices, public benefit agencies, landlords, and schools, based on individual and family needs. Support family members in advocating for themselves.
  • Engage multiple family members in program activities when appropriate and facilitate opportunities for families to build social connections and healthy relationships.
  • Respond to financial requests (rent, gift cards, etc.) appropriately, following established request and approval processes.
  • Maintain up-to-date Release of Information (ROI) documentation annually, ensuring compliance with privacy standards.
