Pay: $20/hour
Full-Time position is eligible for Medical, Dental, Vision, Short Term Disability, Life/Accidental Death and Dismemberment, Flexible Spending Accounts, Long Term Disability, and several voluntary supplemental benefit offerings. In addition, paid time off in the form of vacation, sick, holiday, floating holiday, jury duty, and bereavement leave is available. Job SummaryThe Case Manager II, Programs will assist in the design and delivery of program activities for persons with mental, physical, and/or intellectual/developmental disabilities. This position provides continuous support to Employment and Community based programs and serves as a champion of Goodwill’s values – “FACE IT” (Fun, Accountability, Collaboration, Empowerment, Innovation and Transparency).
Essential Functions- Supervises a caseload of consumers across all disability areas and encourages utilization of multiple community agencies.
- Develops and maintains contact with agency staff and local service providers to create specialized services.
- Assigns, guides, coaches, and trains subordinates in alignment with Community Employment and Day Programs, and Goodwill policy.
- Provides case management information, administrative rules and regulations, community resources, vendors, and programs.
- Serves as a resource for sensitive consumer needs and accommodations.
- Develops and monitors Individual Services and Supports Plans for each consumer and adjusts plans as needed.
- Participates on inter‑disciplinary teams and attends all team meetings.
- Prepares case progress notes, social summaries, and other reports concerning consumer cases.
- Attends staff meetings regarding new or revised policies and procedures.
- Collaborates with and receives direct supervision from Manager when making consumer service decisions.
- Arranges for American Sign Language interpreters as needed.
- Performs any other duties as required or assigned.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Education- A high school diploma or equivalent is preferred. Relevant field or equivalent combination of training and experience in a social service, counseling, or rehabilitation setting is also considered.
- Preferred prior experience working with individuals with intellectual or developmental disabilities.
- Preferred one (1) year of experience as a case manager and/or in administrative positions.
- Excellent written and verbal communication skills.
- Ability to relate effectively to people with disabilities, their families, host home providers, and guardians.
- Ability to work independently, cooperatively, and exercise effective planning.
- Demonstrates flexibility within established deadlines.
- Proficient in writing clear, concise, and accurate reports and plans.
- Knowledge of community resources and ability to program, train, and motivate participants.
- Maintains a professional appearance that enhances the firm’s reputation.
- May require overtime, travel, occasional weekend and night work for residential on‑call system.
- Must have a valid driver’s license and be able to drive for company business.
- Driver insurance requirements: 19–70 years of age for passenger services, 21 years for CDL.
