Hospice Social Worker - Family-Centered End-of- Care

TRU Community Care is seeking a Hospice Medical Social Worker to deliver psychosocial services to patients and families across community, home, and facility settings in Broomfield, Louisville, and Lafayette areas.

You will join an Interdisciplinary Team, supporting end‑of‑life care with compassion, cultural sensitivity, and collaborative care planning. A Master’s in Social Work and CPR certification are required.
