Location: Salida

We are Hiring Registered Nurses to work at our Regional Assessment Center in beautiful Salida, CO! PT, PRN and FT positions available – Night/Weekend Differential – Sun thru Thurs $7/hr additional and Fri – Sat $10/additional.

Full-time sign-on bonus: $6,000 for full time payable 1k at the end of 12 months, 2k at the end of 24 months, and final payment of 3k at the end of 36 months of employment.

Part-time sign-on bonus: $3,000 for part time payable 1,500 at the end of 6 months, 1,500 at the end of 12 months of employment.

General Duties

The Registered Nurse (RN) is responsible for the coordination, provision and oversight of professional nursing care for patients including assessment, treatment, care planning, medical care, care coordination and evaluation. The RN is the primary liaison between patients, staff and other members of the patient's care team.

Essential Duties/Responsibilities
  • Demonstrates quality, confidential and compliant clinical nursing care.
  • Determines the needs of the patient assessing the urgency of connecting the patient to the needed resources based on the patient's condition.
  • Obtains patient consent for care and ensures patient confidentiality.
  • Obtains and documents health history, chief complaint, vital signs and risk factors
  • Assess disease condition.
  • Provides patient care and care coordination to meet the needs of the client.
  • Administers prescribed medications.
  • Collaborates with the patient to determine patient goals, interventions and a process of monitoring progress towards goals.
  • Educates patient and families about health status, maintenance, and management of conditions.
  • Serves as the liaison between the medical and behavioral health staff.
  • May provide supervision to nurses and care coordination staff.
  • Documents patient assessment, goals, intervention and goal completion in the EHR.
  • Serves as key problem-solving resource for staff on patient issues.
  • Helps develop and implement regulatory compliant policies, procedures, quality assurance plans, and work standards.
  • Serves as the RN consult for the ACT team which includes supporting patients and staff who are on that team.
  • Maintains skills to provide direct care.
  • Participates in multidisciplinary teams to improve patient care processes and outcomes.
  • Performs other duties as assigned.

Job Qualifications Knowledge, Skills, and Ability:
  • Knowledge of professional nursing theory, healthcare laws, regulations, and practices.
  • Knowledge of nursing processes; health care systems, structure, and functions
  • Knowledge of health care technology, equipment, and supplies.
  • Knowledge of state law on nursing care, nursing guidelines, and clinic policies and procedures.
  • Knowledge of chart/medical record documentation requirements.
  • Knowledge of federal and state laws related to release of health care information.
  • Knowledge of common safety hazards and precautions to maintain a safe work environment.
  • Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care.
  • Skill in time management, problem solving, multitasking, prioritizing, and care coordination.
  • Skill in initiating appropriate crisis interventions and emergency response.
  • Skill in patient triage in person and on the phone.
  • Skill in preparing/maintaining records, writing reports and responding to correspondence.
  • Skill in exercising high degree of initiative, judgment, discretion, and decision making.
  • Ability to analyze options and counsel patients and families about choices.
  • Ability to engage clients and families in the development and implementation of a care plan.
  • Ability to read and interpret physicians' orders and notes from other providers.
  • Ability to manage the overall provision of nursing services and build consensus among staff.
