Primary Job Purpose

Designated by Administration to have the authority, responsibility, and accountability for the functions, activities, and training of the Nursing team in conjunction with the Director of Nursing. Develops and maintains nursing service objectives, executes neighbor care policies, maintains standards of nursing practice and departmental fiscal responsibility, and collaborate with the CNA Supervisor and Director of Nursing and training for the nursing team while directing the ongoing quality improvement programs.

Job Functions Neighbor Care
  • Practice the Values, the Mission, and Philosophy of Eben Ezer Lutheran Care Center.
  • Determines type and amount of nursing care needed to meet the neighbors needs prior to admission and is involved in any critical decisions involving neighbor care and monitors the admission process.
  • Performs daily rounds and reviews nursing reports to keep current with neighbor care and needs. (Evaluates neighbor needs and correlates care by scheduling daily rounds, reviewing nursing reports & medical records, visiting neighbors, and observing care).
  • Assigns responsibility and delegates’ authority for neighbor care.
  • Continually upgrades the care given through education and maintenance of good communication between neighbors and team members.
  • Participates in discharge planning. Ensures that physician orders are obtained for discharge and prepares neighbor discharge instructions.
  • Cooperates with all departments to maintain a safe and sanitary environment.
  • Manages nursing department budget and reviews monthly departmental financials with NHA and submits variance reports, in absence of the Director of Nursing.
  • Reviews potential admissions as part of the admissions team.
  • Develop and implement an ongoing infection prevention and infection control program to prevent, recognize, and control the onset and spread of infections in order to provide a safe, sanitary, and comfortable home.
  • Establish campus-wide systems for the prevention, identification, reporting, investigation, surveillance and control of infections and communicable diseases of neighbors, staff, and visitors.
  • Develop and implement written policies and procedures in accordance with current standards of practice and recognized guidelines for infection prevention and control.
  • Direct and oversee the antibiotic stewardship program for the care center.
  • Perform analysis of collected data and use this information for infection control efforts, to detect outbreaks, direct in‑service education and identify individual neighbor problems for intervention.
  • Report infection prevention data and improvement efforts monthly to the Continuous Quality Improvement committee.
  • Oversee care activities that increase the risk of infection (i.e., catheter use, wound care, skin care, incontinence care, etc.).
  • Provide education to all staff to assist in preventing the spread of infection on the campus; such as hand hygiene, PPE use, etc.
  • Implement and provide neighbor and employee vaccination programs.
  • Review and revise the infection prevention program, it’s standards, policies and procedures, at least annually and as needed for changes in the annual assessment to ensure they are effective and in accordance with current standards of practice for preventing and controlling infections.
  • Ensure public health, Medical Director, Director of Nursing, and Nursing Home Administrator are notified of any incidence of a reportable disease, infestation or an outbreak on the campus.
  • Manage restorative program and staff.
Personnel
  • Sets standards for qualifications of personnel while following regulations regarding licensed personnel.
  • Responsible for hiring adequate nursing staff.
  • Initiates special focus groups and committees, as necessary to address neighbor care needs.
  • Supervises new team member orientation, supervises and evaluates team members and holds them accountable to written performance standards, in the absence of the D.O.N.
