OPEN POSITION

Minimum Data Set & Staff Development Coordinator (MDS/SDC)

POSITION TYPE

Full Time, 40 hours per week, 8 to 10-hour shifts

POSITION REQUIREMENTS

Colorado or compact Licensed Practical or Registered Nurse, BLS certification

COMPENSATION BASE RATE

$27.00 to $45.50 dependent on experience (includes shift differentials)

$3,000 sign on bonus available to individuals not employed at Hillcrest Care Center & The Towers in the last 36 months, with full-time employment & 1-year commitment.

Job Description

Conduct and coordinate the development and completion of resident assessments in accordance with state and federal regulations, facility policies & goals. Develop, implement, and maintain an effective training program for all new and existing staff, contracted staff, and volunteers, consistent with their expected roles and requirements set forth by state and federal regulations.

Essential Duties include but not limited to
  • Manage resident assessment process
  • Ensure accurate data collection & submissions
  • Care plan development
  • Advocacy & education for residents & families
  • Coordinate & collaborate with healthcare team, residents, and families
  • Participate in quality improvement
  • Facilitate education & training for staff
  • Maintain training & competency records
  • Facilitate employee support & professional development
