About the PositionThe ADON/MDS Coordinator will work within the Senior Care Center’s Leadership Team to ensure accurate and timely completion of Medicare Data Set (MDS) assessments, support quality improvement initiatives, and maintain compliance with state and federal regulations. This role requires close collaboration with the Director of Nursing and interdisciplinary care teams, as well as direct communication with residents, families, and healthcare providers.
PrimeResponsibilities
- Collaborate with the Director of Nursing to lead continuous quality improvement initiatives, promote safe and effective resident care, and uphold all state and federal regulatory standards.
- Coordinate the interdisciplinary team to ensure accurate and timely completion of MDS assessments in accordance with CMS guidelines.
- Communicate with healthcare providers and residents/families to provide timely updates on treatment plans, resident condition changes, and request responses.
- Regularly review and revise care plans to maintain accuracy, support individualized, resident‑centered care, and align with regulatory standards.
- Current RN license in the State of Colorado (minimum).
- Minimum of one year of experience in a hospital, skilled nursing facility, or comparable institution.
- Computer proficiency and understanding of the MDS process; experience with rehabilitative and restorative nursing practices preferred.
- Current BLS certification; completion of an approved MDS course or certification is a plus.
Physical Requirements
Mon‑Fri 8:00 A‑4:00 P, 40 hrs / wk onsite required with rotating call. Physical demands include standing, sitting, changing positions, reaching, traversing stairs, and lifting up to 20 lbs. The role requires clear vision (with corrective eyewear) and hearing, and the ability to handle frequent walking and handling tasks.
Compensation & Benefits- Hourly pay ranging from $38.91 to $49.61, depending on experience.
