Immediate need for a talented LPN/LVN – SNF / LTC
. This is a 06+ Months Contract opportunity with long-term potential and is located in Walsenburg, CO (Onsite).

Pay Range: $45 - $50/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Provide direct nursing care to residents in a skilled nursing/long-term care setting.
• Administer medications and treatments according to provider orders.
• Monitor residents' conditions and report changes to the appropriate healthcare team.
• Perform nursing assessments and document patient care accurately.
• Assist with the development and implementation of resident care plans.
• Collaborate with RNs, CNAs, physicians, and other healthcare professionals.
• Provide education and support to residents and families.
• Maintain accurate and timely clinical documentation.
• Follow infection-control, safety, and facility policies.
• Promote resident comfort, dignity, and safety.

Key Requirements and Technology

Experience:

• BLS Certification – Required
• Previous Long-Term Care/SNF experience preferred
• Previous travel experience required
• Must be comfortable working 12-hour day shifts.
• Must meet all facility compliance requirements before the start date.
