Travel Licensed Practical Nurse Skilled Nursing

Licensed Practical Nurse – Skilled Nursing, based in Wray, Colorado, United States. Travel assignment starts 12/07/2026 for several weeks with guaranteed 40 hours per week. Night shift, 23:00–07:00, five 8-hour shifts. Work setting is a skilled nursing facility.

• Administer medications and treatments per physician orders and facility policy
• Monitor patient conditions, document vitals, and report changes to the supervising RN
• Assist with activities of daily living and support resident safety and comfort
• Perform basic nursing procedures and delegated tasks under supervision
• Maintain accurate medical records and charting in the designated system
• Collaborate with the interdisciplinary team to implement and update care plans

• Active LPN license in Colorado
• CPR/BLS certification
• Prior experience in skilled nursing or long-term care preferred
• Ability to work night shifts and adapt to a fast-paced SNF environment
• Strong communication, documentation, and teamwork skills
• Proficiency with basic computer charting and electronic health records
