The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at‑risk families, children, and individuals throughout Colorado. We advocate for and provide a continuum of housing and services to improve the health, well‑being, and stability of those we serve.

We work to remove barriers that restrict access to these rights and create lasting solutions to homelessness through honoring dignity, building trauma‑informed communities, advocating for equity, continuous quality improvement, and effective resource use.
- Participate in intake, orientation, and engagement of new residents.
- Provide ongoing engagement and assessment to develop and update person‑centered, strengths‑based goals.
- Facilitate access and follow‑up with community‑based primary and behavioral health care providers.
- Meet with each resident on assigned caseload at least twice a month to assess progress, assist with schedules, conduct room checks, review goals, and coordinate staffing.
- Evaluate residents’ needs through regular contact, including crisis intervention as necessary.
- Manage residents from intake to community transition.
- Provide regular group meetings to build community and share resources.
- Assist with managing the community milieu and providing backup case‑management coverage.
- Provide transportation for residents when needed.
- Acquire goods and services (e.g., housing, , benefits, health insurance, food, medication).
- Participate in team and supervisory meetings as assigned.
- Perform additional duties as assigned.
- Bachelor’s degree in social work, counseling, or a related field preferred; relevant experience may substitute for education.
- Experience working with disenfranchised persons is a plus.
- Valid driver’s license required (waiver possible based on overall experience).
- Ability to perform essential job functions with or without reasonable accommodation.
- Choice of HMO or PPO health insurance coverage; full‑time employees pay 1% of earnings for own coverage.

$18.86 – $23.95 per hour. Placement in range depends on total relevant years of experience.
