The Occupational Therapist (OT) is responsible for evaluating, planning, and implementing individualized occupational therapy treatments for residents. This role focuses on enhancing residents’ abilities to perform activities of daily living (ADLs), improve functional mobility, and promote overall independence. The OT collaborates closely with the interdisciplinary care team to ensure high-quality, resident-centered care. A strong understanding of geriatric rehabilitation, compassion, and clinical expertise are essential to support optimal outcomes and quality of life for residents.

- Conduct comprehensive evaluations to assess residents’ physical, cognitive, and psychosocial function.
- Develop individualized treatment plans focused on improving activities of daily living (ADLs), self-care tasks, functional mobility, and safety awareness.
- Implement occupational therapy interventions including therapeutic exercises, ADL training, adaptive equipment use, and task-specific activities.
- Monitor and document residents’ progress, adjusting treatment plans as needed to support optimal outcomes.
- Educate residents, families, and caregivers on treatment plans, home exercises, and strategies to promote independence and safety.
- Collaborate with interdisciplinary team members to ensure coordinated, resident-centered care.
- Maintain accurate and timely documentation of evaluations, treatments, progress notes, and discharge planning.
- Ensure compliance with facility policies, HIPAA, OSHA, infection control protocols, and regulatory standards including PDPM and SNF-specific requirements.
- Participate in quality improvement initiatives and contribute to the development of best practices within the rehabilitation department.
- Stay current with advancements in occupational therapy, geriatric rehabilitation, and regulatory guidelines.
- Engage in continuing education and professional development opportunities.
- Provide training and mentorship to occupational therapy assistants and other team members to support clinical excellence.
- Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state and facility guidelines.
- Provide coaching, development, and corrective action as needed.
- Evaluate performance and offer constructive feedback to support team growth and quality care delivery.
- Master’s or Doctorate degree in Occupational Therapy from an accredited institution
- Current state licensure as an Occupational Therapist.
Minimum 2 years of experience in occupational therapy, preferably in a long-term care or geriatric setting.
Knowledge, Skills, and Abilities- Strong clinical skills in occupational therapy assessment and treatment.
- Strong understanding of Medicare, Medicaid, and PDPM documentation standards
- Excellent communication and interpersonal skills.
- Ability to build and maintain strong relationships with residents, families, and team members.
- Compassionate and patient‑centered approach to care.
- Strong organizational and problem‑solving skills
- Proficiency with electronic medical records (EMR) and Microsoft Office Suite.
- A willingness to learn new technology platforms as needed.
- Ability to work collaboratively within an interdisciplinary team.
