Base Pay Range

$40.00/hr - $50.00/hr

Job Requirements
  • Bachelor (or higher degree) of Science in Physical Therapy from an accredited program
  • Successful completion of National Certification Examination for Registered Physical Therapist
  • Current state licensure or license eligible
  • Assumes responsibility for ongoing continuing education and professional development

Job Relationships

Responsible to and reports to the Director of Therapy/Rehab.

Job Responsibilities
  • Provide and direct physical therapy services to patients, including assessment, treatment, program planning and implementation, documentation, and communication under physician orders.
  • Initiate referrals when appropriate in accordance with Northwoods Rehab policies and procedures.
  • Conduct screening assessments at regular intervals to determine intervention needs.
  • Evaluate patients to obtain data for treatment planning and implementation.
  • Interpret and communicate evaluation findings to patients, families, and other team members.
  • Establish individual patient schedules with other team members.
  • Develop treatment plans, identifying short- and long-term goals and methods to achieve them.
  • Coordinate treatment plans with patients, families, and the therapeutic team.
  • Implement and conduct treatments as outlined in the treatment plan.
  • Monitor patient responses to intervention and modify treatment as indicated to attain goals.
  • Develop appropriate home or community programs to maintain and enhance patient performance in their own environment.
  • Document assessment results, treatment details, follow‑up, and termination of services.
  • Adapt treatment to meet the individual needs of patients.
  • Provide and recommend adaptive devices, orthotics, or prosthetics; train patients, families, and staff in their use.
  • Participate in discharge planning.
  • Supervise and direct Licensed Physical Therapy Assistants in treatment plan implementation.
  • Supervise Physical Therapy Aides, Rehabilitation Techs/Aides, students, and volunteers.
  • Review quality and appropriateness of services delivered and individual physical therapy programs for effectiveness and efficiency using predetermined criteria.
  • Participate in patient, family, and staff education.
  • Coordinate the total rehabilitation program with other team members.
  • Adhere to Northwoods Rehab procedures regarding documentation of physical therapy services.
  • Attend in‑service and facility meetings as required.
  • Provide in‑service education to care team members and the community.
  • Maintain a safe and clean work area, complying with infection control, Universal Precautions and OSHA standards.
  • Demonstrate sound judgment in evaluation, planning, implementation, and follow‑up of patient therapy programs.
  • Maintain positive relationships and rapport with coworkers, supervisors, patients, family members, and facility personnel.
  • Report to work on time and coordinate schedule for efficient patient treatments.
  • Assist with and participate in the Quality Assurance Program as required.
  • Adhere to established confidentiality standards.
  • Project a positive and professional image at all times.
  • Travel for home assessments within a reasonable distance from the facility if required.
  • Maintain secure passwords for the required software.
  • Perform other duties as assigned to effectively fulfill role responsibilities.
