Role OverviewWe are seeking a skilled and compassionate Physician to join Legacy Care on a PRN basis in Columbia, MD. This role involves providing high‑quality primary and acute medical care within post‑acute and long‑term care facilities, collaborating with multidisciplinary teams to ensure safe, person‑centered care and effective transitions for residents.

- Conduct comprehensive assessments and develop individualized care plans for residents in post‑acute and long‑term care settings
- Provide diagnosis, treatment, medication management and follow‑up care consistent with current evidence‑based practice
- Respond to acute changes in condition, coordinate timely interventions and, when appropriate, facilitate transfers of care
- Collaborate with nursing staff, advanced practice clinicians, therapists, social workers and facility leadership to promote holistic, person‑centered care
- Participate in facility rounds, care‑planning meetings and interdisciplinary case conferences
- Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
- Support medication safety, antimicrobial stewardship and quality improvement initiatives
- Provide clinical consultation on complex cases and mentor facility staff and medical colleagues as needed
Qualifications & Skills – Physician, Columbia MD
- Medical degree (MD or DO) from an accredited institution
- Active and unrestricted Maryland medical license (or eligible for licensure)
- Board certification or board eligibility in Family Medicine, Internal Medicine
- Clinical experience in post‑acute care, long‑term care, geriatrics or related settings preferred
- Proficiency with Electronic Medical Record systems and clinical documentation
- Strong clinical judgement, interpersonal and communication skills; ability to work effectively within multidisciplinary teams
- Commitment to patient‑centered care, professional integrity and ethical practice
