Occupational Therapist, Home Health
Join Center Well Home Health as an Occupational Therapist in our Home Health team. You will help patients regain strength, mobility, and independence from their homes.
Pay Range- Base pay: $84,900 – $116,800 per year
- Per visit/unit: $54.00 – $75.00
- Assess/screen patient daily living/work‑related skills and develop therapeutic retraining programs with measurable objectives.
- Administer and interpret diagnostic and prognostic tests of function; assist physician in evaluating patient’s level of function.
- Collaborate with physicians and health‑care team to develop and revise Plans of Care.
- Provide therapeutic treatment and instruction to patients to improve/restore strength, coordination, range‑of‑motion and function.
- Review and expand retraining programs in a manner consistent with patient goals and rehabilitation guidelines.
- Consult with vocational team members to develop and implement vocationally oriented plans when appropriate.
- Report patient status and progress to physician, rehabilitation staff, Clinical Manager, and families.
- Participate in care coordination and discharge planning; act as resource for other health‑care personnel.
- Design community reintegration activities to assist physical reconditioning and psychological adjustment.
- Recommend and design special adaptive equipment to enhance patient participation.
- Train staff and family members to carry out the retraining program (including dressing, feeding, grooming, hygiene).
- Coordinate, oversee, supervise, and instruct Occupational Therapy Assistants and Home‑Health Aides.
- Accurately document patients’ care observations, interventions, and evaluations; submit treatment plans and discharge summaries.
- Degree from an accredited Occupational Therapy program.
- Minimum of six months occupational therapy experience (preferred).
- Home Health experience a plus.
- Current, unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver’s license with auto insurance and reliable transportation.
