Licensed Practical Nurse I at Family First Health. Family First Health is a community‑focused federally qualified health center providing accessible primary, dental, and social services across York, Adams, Lancaster, and Lebanon counties. Essential Duties and Responsibilities- Collaborate with the Patient Care Team to provide clinical direction and support in accordance with FFH procedures.
- Work at multiple locations as scheduled, documenting patient medical information to enhance provider ability to deliver comprehensive care.
- Obtain and electronically record vital data, medication requests, allergies, and chief complaints during patient visits; prepare exam rooms and sanitize rooms after treatment.
- Accurately administer medications (insulin, injections, immunizations) with peer‑checking technique; collect, label, and document laboratory samples; provide educational material to patients.
- Follow HIPAA procedures, verify patient identity, and maintain confidentiality.
- Handle, store, and maintain vaccines, documenting accurately at all points in the electronic record.
- Communicate effectively with the healthcare team, patients, and families to ensure excellent service.
- Support team at other sites, complete incident and complaint reports, perform nurse visits per policy, and maintain medical supply inventory.
- Perform opening and closing lab procedures and collaborate with the supervisor to identify areas needing improvement.
- Retrieve documents, follow clinical algorithms, schedule and arrange appointments, process prescription requests, triage and refer patients under licensed staff supervision.
- Complete prior authorization forms, coordinate pre‑authorizations with insurance carriers, and document referrals as required in the electronic health record.
- Assist in meeting meaningful use and patient‑centered medical home certification requirements.
- Perform all other duties as assigned.
- Active Pennsylvania licensure for a Licensed Practical Nurse.
- Current CPR and Narcan certification.
- Proficiency with computer software (Microsoft Office and related applications).
- Excellent customer service skills and strong verbal and written communication.
- Basic arithmetic and ability to make accurate calculations and reconcile figures.
- Critical thinking, problem resolution, and effective teamwork abilities.
- Ability to travel to multiple sites, work in a demanding environment, and juggle multiple competing tasks.
- Adherence to Family First Health policies, including code of conduct and workplace conduct.
- Ability to articulate and advocate Family First Health’s mission and values.
