We are a neuro rehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care.

Make an Impact in our Home and Community Program. We are looking for an experienced Speech‑Language Pathologist (SLP) enthusiastic about providing functional rehabilitation wherever life happens—at home, school, work, or in the community.

• Perform evaluations and develop treatment plans for patients with speech, language, cognitive, and swallowing disorders.
• Regularly reassess treatment effectiveness, attend staffing meetings, and participate in family conferences.
• Perform discharge evaluations and provide recommendations for home and community safety, including referrals and education resources.
• Communicate patient needs and progress to the treatment team, physicians, service recipients, and family members.
• Collaborate with other disciplines to ensure coordinated care and enhance patient outcomes.

• Master’s Degree from an accredited Speech‑Language Pathology program.
• Valid Speech‑Language Pathology license in the state(s) of practice.
• Current CPR certification in accordance with state regulations.
• Minimum of one year’s work experience as an SLP.
• Knowledge of rehabilitation techniques related to complex neurological injury preferred.
• Effective verbal and written communication skills.
• Ability to lift 50 pounds; lifting of up to 100 pounds may be required following safety procedures.
• Fine motor skills, visual acuity, and ability to walk/stand for extended periods.
• Physical tasks such as pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently.
• Health screening may be required prior to assignment and periodically thereafter.
