Monogram Health is seeking a Market Physician Executive (MPE) to lead our in‑home multi‑specialty polychronic care model in Columbus, GA.
Position OverviewThe MPE will oversee daily clinical and business operations, collaborate with community physicians, manage a multi‑disciplinary practice team, and focus on improving patient experience, outcomes, provider satisfaction, and cost reduction.
Roles and Responsibilities Clinical Guidelines Execution – 25%- Know, understand, and deliver Monogram Health’s proprietary evidence‑based clinical pathways.
- Ensure adherence to established clinical guidelines and the Monogram Model of Care.
- Review and approve APP, RN, SW, and Pharm
D plans of care. - Appropriate and timely patient documentation within Salesforce and Athena clinical activities.
- Co‑sign APP encounters.
- Accountability for reducing total cost of care and Medical Loss Ratio.
- Responsible for clinical outcomes including utilization, prescribing, and HEDIS/GAP closure.
- Lead daily high‑risk and concurrent review rounds.
- Direct supervision of front‑line clinical and operations team members.
- Oversee and delegate operational responsibilities to Market Manager for patient engagement, scheduling, admin oversight, strategic implementations, and P&L management.
- Regularly assess and present market performance to executive leaders.
- Provide direct and indirect patient care, including diagnosis and treatment of disease.
- Engage patients on treatment plans and evidence‑based care pathways.
- Conduct peer‑to‑peer collaboration with community, facility, and health plan partners.
- Order labs, referrals, and complete actions to drive patient outcomes and close care gaps.
- Provide clinical guidance and direction to drive population health activities.
- Identify and intervene on high‑risk patients, formulate strategies to reduce admissions/readmissions.
- Complete quality post‑hospital discharge visits and improve visit quality.
- Collaborate with Medical Economics, Finance, and other stakeholders to root cause utilization trends.
- Present population health strategies in clinical and operational meetings.
- Conduct patient reviews to target high utilizers and high‑opportunity cohorts.
- Participate in Monogram on‑call activities; minimum 7 days on call per quarter.
- Provide coverage for other MPEs during PTO or vacancy as needed.
- Willing and able to obtain hospital privileges at required facilities.
- Remote within the designated market with occasional in‑home patient visits and domestic travel.
- Demonstrated experience applying evidence‑based clinical criteria.
- Experience in renal care and geriatrics.
- Strong management and communication skills.
- Active, unrestricted state medical license in each state within the market, free of sanctions from Medicaid or Medicare, and willingness to obtain licensing in multiple states.
- MD or DO degree from an accredited medical school.
- BC or BE in an ACGME‑approved specialty such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics.
- Experience with Medicare Advantage and Managed Medicaid populations.
- Experience with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment.
