Join Center Well Home Health as an Occupational Therapist in a home health setting. You will assist patients in regaining strength, mobility, and independence in the comfort of their homes.
Responsibilities- Assess and screen patient daily living and work‑related skills; develop therapeutic retraining programs with measurable objectives.
- Administer and interpret diagnostic and prognostic tests of function in patients’ homes or other settings.
- Confer with physicians and the healthcare team to develop or revise the Plan of Care Treatment.
- Provide therapeutic treatment and instruction to patients according to physician orders to improve strength, coordination, range‑of‑motion, and function or teach compensation measures.
- Review and expand retraining programs consistent with behavioral goals and rehab program guidelines.
- Consult with vocational team members to develop and implement vocationally oriented plans for patients.
- Report patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients, and families.
- Participate in care coordination, discharge planning, and act as a resource to other health personnel.
- Design community reintegration activities to assist patients in physical reconditioning and psychological adjustment, coordinating with the interdisciplinary team.
- Recommend or design special adaptive equipment to improve residence or working environments or participation in rehabilitation or community.
- Design and train staff and family members to carry out the retraining program, including dressing, feeding, grooming and hygiene skills, and participate in team and family educational meetings.
- Coordinate, supervise, and evaluate Occupational Therapy Assistant and Home Health Aide performance implementing occupational therapy services.
- Accurately document patients' care observations, interventions, and evaluations during service delivery; submit evaluation, treatment plans, and discharge summary to the supervisor and care management staff.
- Degree from an accredited occupational therapy program.
- Minimum six months occupational therapy experience preferred; home health experience a plus.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver’s license, auto insurance, and reliable transportation.
- Hourly: $49.00 – $69.00 per visit/unit.
- Base salary: $77,200 – $106,200 per year.
