RN Hospice Case Manager *$10,000 Sign On Bonus Eligible! (Columbus, GA.) Pro Health Home Health and Hospice

Pro Health Home Health and Hospice is hiring an RN Hospice Case Manager to provide nursing care to terminally ill hospice patients and support their families.

Job Summary

To provide nursing care to the terminally ill hospice patient as needed, assist families, and work as a member of the hospice team.

Qualifications
  • Graduate from an accredited school of nursing.
  • Current license as a registered nurse in the state.
  • Minimum 2 years of experience as a registered nurse; active patient contact within the past 3 years preferred.
  • Completion of hospice training program.
  • Ability to work independently, make accurate and timely judgments, supervise others, respond to crisis outside hospital, adapt to diverse social, racial, cultural, and religious contexts.
  • Strong interpersonal skills, emotional stability, and ability to cope with mental and emotional stress.
Physical Requirements
  • Require considerable physical effort: kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes, and maximal assistance in lifting/transferring up to a 100-pound patient.
  • Must possess sight/hearing or use adaptive devices adequate for duties.
  • Must provide evidence of annual TB test and other state-required tests or examinations.
Mental Requirements
  • Work independently, make judgments based on assessments and data.
  • Be flexible, innovative, possess good interpersonal skills.
  • Ability to cope with mental and emotional stress, demonstrate emotional stability.
Working Conditions

Tolerate exposure to elements including odors, blood, body fluids, excrements, adverse environmental conditions, and hazardous materials.

Transportation

Must have a current valid driver’s license, auto liability insurance and reliable transportation.

Essential Functions
  • Regularly assesses and reassesses the nursing needs of the hospice patient.
  • Provides dietary counseling.
  • Provides hospice nursing services, treatments and preventive procedures.
  • Initiates nursing procedures appropriate for the patient’s hospice care and safety.
  • Observes signs/symptoms and reports to the physician and IDG members any unexpected changes in the patient’s condition.
  • Teaches, supervises, and counsels the hospice patient and family members about providing care.
  • Supervises and trains other nursing service personnel.
  • Develops and re-evaluates the patient/family care plan with IDG to meet needs and maintain continuity of care.
  • Performs specific nursing procedures as needed following doctor’s orders.
  • Attends team conferences.
  • Maintains records as required by hospice.
  • Follows hospice policies and procedures, observes confidentiality and safeguards patient information per HIPAA.
  • Communicates to supervisor if unable to meet patient needs or perform a procedure.
  • Participates in on-call system and covers duties when unavailable for assigned duties.
  • Maintains skills and knowledge.
  • Works with interdisciplinary group for patient care.
  • Coordinates implementation of care plan for patients residing in SNF, NF, ICF or MR.
  • Organizes work schedule and uses time management to attend meetings.
  • Complies with agency infection control policies and protocols.
  • Assists with orientation, teaching, and training as requested.
  • Other duties as assigned by the Director.
