- Provide nursing care and complete assessments of patient/family needs, including physical, emotional, and bereavement status.
- Offer consultation and education on end‑of‑life issues and palliative care.
- Educate partner physicians, hospital staff, and social service organizations.
- Collaborate with referral sources and coordinate palliative care consults.
- Prepare patients for provider visits, including vital signs and record updates.
- Assist with routine physical exams and maintain confidentiality at all times.
- Participate in organizational activities and committees as appropriate.
- Associate or Bachelor Degree in Nursing (ASN/BSN).
- Current Registered Nurse License in Indiana.
- One to three years of experience in health care (preferred).
Tuesday‑Friday, 7 am – 5:30 pm.
