Job Description Summary

Works with the multidisciplinary patient care team to provide optimal pharmaceutical care to patients in a focused practice area. Demonstrates competency across all areas of pediatric clinical pharmacy practice and provides age‑appropriate care to patients from birth through 21 years and select adult patient populations. Routinely participates in teaching, research, quality improvement endeavors, and scholarly activities.

Essential Functions
  • Serves as a pharmacotherapy expert to prospectively recommend and evaluate individualized drug therapy. Provides therapeutic drug monitoring and recommends evidence‑based patient‑specific actions to achieve best outcomes, while considering pharmacoeconomic principles. Prospectively identifies potential adverse drug reactions, drug‑drug, drug‑disease, and drug‑food interactions, and incompatibilities affecting patient outcomes.
  • Documents direct patient care, clinical, operational, and financial outcomes and activities appropriately in the medical record.
  • Provides appropriate handoff for pharmacists and other healthcare providers to follow up on anticipated pharmacotherapy needs, including medication changes. Collaborates to assure continuity of care and consistency of practice.
  • Focuses on accurate maintenance of the medication list and ensures accurate and timely communication regarding medication‑related needs. Participates in the development and coordination of patient‑specific teaching and transitional care services as part of medication reconciliation.
  • Precepts PGY2 and PGY1 residents, Doctor of Pharmacy students, and Pediatric Pharmacy Fellows. Develops, provides, and participates in pharmacy staff development activities.
  • Provides team‑based education and training to medical staff, fellows, residents, and students. Participates in research and scholarly activities as an investigator or consultant to the pharmacy, medical, and nursing staff. Routinely contributes and provides external scholarship through publication of articles in peer‑reviewed journals or conference abstracts.
  • Participates in committees related to medication safety and quality improvement initiatives and demonstrates pharmacy practice leadership.
Education Requirements
  • Bachelor’s Degree from an accredited college of pharmacy, required.
  • Doctor of Pharmacy, preferred.
Licensure Requirements

Licensure, or licensure‑eligibility, to practice pharmacy in the state of Ohio.

Certifications

BPS Board Certification (or eligible for testing, with certification to be obtained within 24 months of hire).

Experience
  • PGY1 and PGY2 pharmacy residencies (or PGY1 residency and PGY2 fellowship) OR PGY1 residency with 3 years of hospital pharmacist practice experience (pediatrics preferred) with responsibility for direct clinical patient care similar to that provided through a PGY2 residency.
  • OR 5 years of hospital pharmacist practice experience (pediatrics preferred) with responsibility for direct clinical patient care similar to that provided through a PGY2 residency.
Physical Requirements

Occasionally:
Bend/twist, biohazard waste, blood and/or bodily fluids, climb stairs/ladder, lifting/carrying 0‑10 lbs, 11‑20 lbs, 21‑40 lbs, pushing/pulling 0‑25 lbs, 26‑40 lbs, reach above shoulder, squat/kneel, walk. Frequently: communicable disease handling, neck flexion/extension, repetitive hand/arm use, sitting, standing. Continuously: audible speech, chemicals/medications, color vision, computer skills, decision making, depth perception, hearing acuity, interpreting data, peripheral vision, problem solving, seeing ‑ far/near.
