Direct Support Professional Home Coordinator
We are looking for a Full-Time Home Coordinator in Columbus, OH.
Job OverviewThe Home Coordinator is responsible for providing direction and guidance to direct care staff caring for clients with intellectual and/or developmental disabilities in a supported living or community-based environment. This includes responsibility for the activities of staff and the whereabouts of all clients under their supervision at all times, as delineated in the ISP, and ensuring that required documentation is completed in an accurate, thorough and timely manner.
Incumbents are on call twenty‑four hours a day and must respond in a timely manner to unexpected events and/or emergencies with clients under their care. The Home Coordinator is also expected to provide direct client care.
- Plans, organizes, and directs the care of clients.
- Supports outcomes and supervision of the individual to ensure supports are in adherence with the person‑centered plan.
- Monitors the client care activities of direct support staff.
- Monitors Individual Support Plans and delivery of client care plans as appropriate, takes actions to ensure Standards of Care are met and participates in quality improvement process.
- Assists with determining staffing schedules, staffing levels, and staff schedules in coordination with the Program Manager.
- Assures that staffing patterns developed are in adherence with the team recommendation and PAWS Allocation.
- Prevents client abuse by adhering to all policies and procedures, attending training, reporting suspicious behavior, and following mandatory abuse reporting requirements.
- Completes administrative and clerical documentation including reviewing daily logs and notes detailing client activities supporting the approved ISP that support services billed through Medicaid HCB Waiver Program.
- Ensures safe operation of all equipment in the house by completing routine site monitoring reports.
- Plans and organizes daily activities in accordance with Individual Support Plan.
- Provides direct client care, including transportation, maintenance of household inventory, menu planning and preparation, etc.
- Provides weekly oversight of individuals’ finances by assisting with managing personal funds and reporting signs of misappropriation or exploitation.
- Maintains and schedules medical appointments and completes all follow‑ups.
- Other duties as assigned.
- High School Diploma or GED required.
- Preferably one year supervisory experience working with developmentally, intellectually and/or physically disabled individuals.
- Required to complete 60 course hours of DODD training for competency.
- Completion of accepted Ohio medication certification and Ohio Developmental Disabilities (DODDs) eight‑hour training course.
- Valid Ohio Driver’s License.
- Valid automobile insurance.
- Proof of completed High School or GED (Diploma or official transcript).
- CPR and First Aid.
- Extensive mental activities: use judgment, advise, counsel, influence, negotiate, organize, plan, and synthesize concepts.
- Ability to work in a demanding environment and make wise decisions under stress.
- Exceptional coaching and mentoring skills to improve employee development.
- Proficient in Microsoft Office (Word, Excel) and equipment such as PC, calculator, copier, fax machine, scanner.
- Aware of physical demands and work environment characteristics. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
- Sitting 50% of work time.
- Standing and walking 30% of work time.
- Bending, pushing, pulling, stooping, reaching, and kneeling 30% of work time.
- Grasping and finger/hand manipulation 50% of work time.
- Ability to lift up to 50 pounds.
- Ability to operate computer, facsimile, copier, shredder, and calculator.
- Office setting with varying noise, standard lighting, ventilation, occasional temperature fluctuations.
- Normal business hours with flexibility to work overtime or non‑standard hours as required to maintain staffing.
