The Community Health Worker (CHW) is a frontline public health worker who is a trusted team member with an in-depth understanding of the community served. The OSU Total Health Care Center (THCC) primary care clinic, a Federally Qualified Health Center, is adding a CHW to our team to serve as a link between health and social services and the community to better facilitate access to services and improve the quality and cultural competence of these services.
The CHW builds individual and community capacity and awareness by increasing health knowledge and self‑sufficiency through a range of activities, such as outreach, community education, intensive case management, social support, and advocacy. The CHW assists patients in accessing appropriate resources, coordinates referrals as necessary, provides community and educational support, and serves as a community liaison.
The CHW will serve as a frontline public health worker, providing outreach, education, case management, social support, and advocacy to community members and participating in coordination with health and social services agencies. The role includes identifying community health needs, developing and delivering culturally appropriate educational materials, and connecting patients to resources and referrals.
Required Qualifications- High school graduate or equivalent.
- Current Community Health Worker certification in the state of Ohio.
- At least 1 year of relevant experience.
- Compassionate and competent in working with diverse cultural, religious, and socioeconomic groups.
- Strong ability to serve as a community liaison between target populations, agencies, providers, and team.
