Schedule: 8 hour shift Monday thru Friday

Responsibilities & Duties:

• Work closely with hospital staff including discharge planners, physicians, social workers, case managers, inpatient units and administration to provide direct and indirect care to clients.
• Work with hospital social worker to begin the housing process if needed.
• Develop and maintain new and current relationships with assigned hospital(s).
• Provide initial and ongoing diagnostic assessments of both hospital and agency clients in the hospital and for the agency and complete the Hospital ISP.
• Provide behavioral health counseling and CPST for clients through home, office and hospital visits.
• Continue to work with clients for up to three months upon discharge to ensure proper and complete linkage with resources and continuity of care.
• Attend treatment team meetings.

Qualifications:

• LISW/LPCC or higher required.
• Previous experience in social work or counseling.
• Strong documentation skills and ability to use electronic health record system.
• Compassionate and caring demeanor.
• Ability to build rapport with clients.
• Strong leadership qualities.
• Excellent written and verbal communication skills.
• Valid driver’s license & insurance.
• Pass background check and drug test.
