The Medication Management / Transitions of Care (TOC) Pharmacy Technician provides a valuable patient‑centered service focused on facilitating transitions of care. Responsibilities include obtaining and documenting a complete and accurate admission medication history and allergy information in the electronic medical record (EMR) and assisting with the discharge medication process. The TOC Technician completes the admission medication history during inpatient admissions, ambulatory encounters, pre‑admission testing, surgery encounters, or within procedural areas under the supervision of a pharmacist, physician, or advanced practice provider.
Information is gathered from face‑to‑face or telehealth interviews with patients, and from communication with patients, caregivers, physicians, retail pharmacies, and long‑term care facilities. The Technician ensures documented information is complete and accurate by identifying discrepancies or escalating to a pharmacist or provider for follow‑up. Throughout the hospital admission, the Technician collaborates with patients and the care team to facilitate discharge by communicating medication options, obtaining credit card information for Meds to Beds delivery, and accessing coverage for medication or engaging the appropriate staff for assistance programs.
The Technician promotes a culture of safety by adhering to all hospital and department rules, regulations, and requirements, including those outlined in the Medication List Documentation and Reconciliation policy, education videos, and Standard Operating Procedures. The Technician participates in continuing educational opportunities to meet annual competency requirements and in departmental training to develop ongoing skills in other pharmacy areas. The Technician acts as a positive role model for all TOC pharmacy technicians and contributes to the hospital’s vision and values.
PositionSummary
The TOC Pharmacy Technician is responsible for obtaining a complete and accurate admission or ambulatory medication history and allergy information, and for helping to streamline and facilitate the discharge medication delivery process when applicable.
Minimum Qualifications- High School graduate or equivalent
- Certified Pharmacy Technician
- 2 years of relevant experience required; 4–6 years of relevant experience preferred
