, Columbus, OH — $15.00 - $18.00 per hour — Full-time | Part-time

DHM is a Joint Commission accredited home health agency with CMS deemed status, dedicated to providing exceptional clinical care to patients in their homes throughout the Columbus area. We are seeking compassionate and an experienced Home Health Aides to join our team. As an HHA with our agency, you ll provide essential personal care services to patients in their homes, working collaboratively with our interdisciplinary care team to ensure the highest quality of patient outcomes.

• Provide personal care services including bathing, grooming, dressing, and toileting assistance
• Assist with transfers, positioning, and ambulation
• Perform vital signs monitoring and reporting
• Assist with medication reminders as directed by the care plan
• Maintain accurate and timely documentation
• Communicate effectively with patients, families, and clinical team members
• Follow infection control and safety protocols

Required:

• Current Ohio HHA certification or STNA certification
• Minimum 75 hours of certified training
• Valid Ohio driver s license and reliable transportation
• Clear background check and drug screening
• CPR/BLS certification
• Ability to lift up to 50 lbs and perform physical care tasks

Preferred:

• Previous home health experience
• Experience working in CMS-regulated environments
• Strong communication and documentation skills
