Psych Pros, Inc. has a full-time, temporary Medical Social Worker assignment available with hospital system in Los Angeles, CA, with travel required to skilled nursing facilities in Los Angeles County.
- Assist patients and their families with care planning and discharge planning.
- Complete psychosocial assessments.
- Partner with patients to identify needs and develop and implement individualized treatment plans based on mutually agreed-upon goals.
- Discuss options for care proactively, including medical center resources and external community/government resources to assist patients and families in developing short- and long-term care plans as appropriate.
- A master's degree in social work.
- Active BBS registration as an ASW.
- One (1) year of recent internship experience in a healthcare setting.
- Hospital acute care experience preferred.
