Serves as a health promotion advocate in the Emergency Department’s Health Now Program, a population health‑focused program using a systemic approach to address health needs proactively. Participates in identifying and intervening with populations most at risk for threats to health and well‑being at the earliest opportunity, helping them connect with community resources to improve outcomes. Under program leadership and patient care teams, performs a wide range of clinical‑support and administrative functions.
Key Responsibilities- Screen and receive referrals for potentially eligible patients for the Health Now program and related services.
- Interview patients to elicit health‑needs history and administer questionnaires in a courteous, professional manner.
- Coordinate point‑of‑care testing and inform patients about relevant clinical services.
- Refer patients to appropriate Health Now Program team members or community resources; maintain patient records and strict confidentiality.
- Maintain tracking and reporting systems; collect, analyze, and compile data for progress reports.
- Assist with organization, maintenance, and timely data entry for clinical research studies.
- Support multifaceted research activities, including participant recruitment, informed‑consent procedures, data collection, and data analysis, as described below.
- Address patient concerns through active listening and escalation to the care team as needed.
- Research available services/programs within and outside the OSU network to match patient needs with resources.
- Collaborate with multidisciplinary teams and health‑care team members to ensure referrals and needs assessments are completed.
- Employ motivational interviewing, open‑ended questions, and client‑centered counseling to collect patient information.
- Recruit study participants and support registration and enrollment processes.
- Assist with informed‑consent procedures, cognitive testing, and collection of clinical data.
- Enter and maintain study data in electronic data‑capture systems and medical records.
- Assist with data audits, protocol file preparation, and drafting summary documents for protocol evaluation, abstracts, and manuscripts.
- Bachelor’s degree or equivalent experience.
- Community Health Worker (CHW) experience preferred but not required.
Final candidates are subject to successful completion of a background check; a drug screen or physical may be required during the post‑offer process.
