Summary:

*** JOB SUMMARYOhioHealth is seeking a pharmacist to join the Heart Disease Management team. This pharmacist will provide evidence-based therapy management and care for patients with pulmonary hypertension as well as other heart conditions (heart failure, LVADs, etc). They will work alongside an interdisciplinary care team in to provide safe, high quality, and cost-effective drug therapy. The pharmacist is responsible for patient-centered education throughout the course of their therapy, providing appropriate patient instruction and dosing guidance based upon tests and laboratory results, reviewing individualized patient treatment plans, and ensuring patient adherence to medication therapy.
This position will support primarily ambulatory needs but have limited inpatient responsibilities related to transitions of care and/or medication use work. Pharmacists will be expected to serve as preceptors for pharmacy students and pharmacy residents in their area of training and expertise.
SPECIALIZED KNOWLEDGEOptimal candidates will have a PGY2 in ambulatory care and BPS certification in ambulatory care, cardiology, or pharmacotherapy or equivalent clinical experience.
MINIMUM QUALIFICATIONS
- Bachelor of Science or Doctor of Pharmacy degree from an accredited institution
- Current Ohio pharmacist license
- Basic Life Support Certification—current or obtained during the orientation period (unless greater than 50% of scheduled work hours, as determined by the manager, are completed off-site or not in a patient care setting)
SPECIALIZED KNOWLEDGE
- Pharmacokinetic drug dosing experience
- Drug therapy management DESIRED ATTRIBUTES- ASHP accredited residency or equivalent pharmacy practice experience
- Pharmacy Board Certifications or other certifications
- Advanced-level degree in healthcare field
- Collaborative practice experience
- Advanced Cardiac Life Support Certification (may be required to obtain as determined by manager)

Acute Care/Infusion
50% - Reviews provider orders for correct drug choice, dosage form, dosing schedule, and possible drug-drug interactions or allergies. Contacts providers to clarify orders as appropriate. Dispenses and compounds medications and dispenses drug therapy-related devices in accordance with standards of practice and Ohio Health pharmacy policies. Ensures all medications, chemicals and supplies are dispensed without suboptimal integrity. Ensures appropriate labeling for proper administration of medication.
Interprets prescriptions and determines compatibility in accordance with standards of practice. Prepares care plan for patients to ensure optimal therapeutic outcomes and maximize compliance with therapy instructions.
